I asked Patrick’s Motors on YouTube this exact question. His response:
“In 2006 a BM carrier came off the line at a resort in Ontario seriously injuring a skier. Prior there had been issues with cracks developing. Shortly after a bulletin came out that involved adding plates to several areas of hanger arm and chair to reinforce them with the understanding that this was a temporary fix and new carriers should be sourced. 1996 and up BM quad carriers are still running at many resorts as this was a entirely new design though they still require 100% NDT every year.”

the original BM brand chairs must have had a number of design & engineering flaws. Oh and what resort did the incident occur at?
